This page is for adults across Central Oregon who have tried something for depression, found it did not work, and are being shown a page of last resorts.

When to get evaluated

When adults in Central Oregon come in about depression

Most people arriving here have already tried something.

One medication, stopped early

Started, judged after two or three weeks, and abandoned. That is the most common reason a workable option gets written off as a failure.

The dose never moved

Months on the starting dose with nobody reviewing it. A medication that was never raised to an effective dose has not really been tried.

The diagnosis was never revisited

Treatment aimed at depression that is not responding is sometimes treatment aimed at the wrong condition.

You are being sold step four

Every result offers something for treatment-resistant depression, and nobody has checked whether the earlier steps were actually completed.

Central Oregon sells the last resort first

This is the most one-sided set of search results of the four markets this practice serves. Seven of the results across the first two pages for depression treatment in this area are TMS clinics or TMS directories, alongside a ketamine clinic and a post about vagus nerve stimulation. Google’s own list of commonly asked questions here is entirely about failure: what to do if you have severe depression and nothing works, when TMS does not work, and what the next step is if antidepressants have not worked.

Those treatments are real and they matter. For genuine treatment-resistant depression they can work when nothing else has, and where that is the right answer this practice will say so and refer. The problem is the order. Almost all of them are indicated after standard treatments have failed, and the page is selling them to everyone.

Before you are treatment-resistant, three things have to be true, and for a lot of people at least one of them is not. The medication has to have been taken at an effective dose, not just a starting dose. It has to have been given a fair length of time, which is closer to six to eight weeks than to two. And the diagnosis it was aimed at has to have been the right one, because depression that sits on top of an untreated sleep disorder, a thyroid problem, ongoing substance use or a different psychiatric condition does not respond to treatment for depression alone.

Severity is a defined thing rather than a feeling, too. The National Institute of Mental Health, using the 2021 National Survey on Drug Use and Health, estimates that 14.5 million US adults had a major depressive episode with severe impairment in the past year, 5.7% of all adults, out of 21.0 million with an episode of any severity. Severe is a subset, not a synonym for not better yet.

Treatment options

What treatment usually looks like, and how long it takes

For most depression the first-line options are antidepressant medication, structured psychotherapy, or both together, and the two are roughly comparable for moderate depression while doing better in combination. Which one fits follows the diagnosis and the severity rather than a preference stated in advance.

The timeline is the thing most people are not told, and it is where most treatment goes wrong. Some effect often appears at two to four weeks. A fair judgement of whether a medication is working is closer to six to eight weeks, at a dose that was actually raised to an effective level rather than left where it started. Deciding at three weeks that something has failed is the most common reason a workable option gets abandoned.

Several conditions produce depressive symptoms without being depression. Thyroid problems, sleep apnea and chronic sleep debt, anemia, alcohol and substance use, and the side effects of medication already being taken all belong on that list, and so do other psychiatric conditions that need a different treatment entirely. Those get checked before anything is prescribed, because treating the wrong one costs months.

Brief techniques such as CBT and motivational interviewing happen inside the same appointment as medication management. Standalone talk therapy without medication management is not offered here, and where ongoing therapy is the right addition that is said plainly rather than worked around.

What to expect

From booking to a plan, in four steps

Scheduling and appointments both run through Headway, which verifies your coverage and hosts the visit. Registering is free.

  1. Check coverage and book

Open Headway, confirm what your plan covers, and self-schedule. No callback queue and no eligibility surprise afterwards.

  2. The evaluation

About an hour rather than a fifteen-minute intake. History, what the depression is stopping you doing, what has already been tried and for how long, and the conditions that imitate it.

  3. The diagnosis and the plan

You get a named diagnosis and a graded severity, then the options that fit it, including an honest read on whether outpatient care is the right level for you.

  4. Follow-ups and adjustment

Closer together early while a dose is settled, then a review every few months. Same prescriber throughout, so nothing is re-explained from the beginning.

Why patients choose this practice

What gets checked before anyone calls it resistant

What was actually tried, and how

Which medications, at what doses, for how long, and why each one stopped. A list of names is not a treatment history and it is usually all anyone has.

Whether the diagnosis holds

Depression that is not responding is sometimes a different condition. That gets re-examined rather than inherited from whoever made the call first.

Questions

Depression questions we get from Sisters

What are the most effective treatments for depression in adults in Bend?

For most adults, antidepressant medication, therapy, or both. When those fail, specialty options like TMS appear in Central Oregon search results. This practice does outpatient evaluation and medication management by video; it does not offer TMS.

When doesn't TMS work for depression in Central Oregon?

TMS is a specialty procedure for treatment-resistant depression and it is not offered here. If your evaluation points that way, you will be pointed to the clinics that provide it in Bend.

What is the next step in Sisters if antidepressants have not worked?

An evaluation that reviews what was tried, doses, duration and diagnosis. Sometimes the next step is a different medication class; sometimes it is clarifying that the diagnosis was incomplete.

Is the Sisters office somewhere I attend for depression appointments?
